New Clients are encouraged to arrive 10-15 minutes early in order to complete their health intake forms prior to their session.
Late Arrivals may have their sessions shortened in order to accomodate other's whose appointments follow yours. Depending on how late you arrive, it may not be possible to provide a sufficient treatment. Should you agree to a shortened session due to lateness, you will still be responsible for the session's full cost. Please plan accordingly and try to be on time.
Cancellations. 18 hours advance notice is required when cancelling an appointment. The scheduling service will send a complimentary reminder of your appointment 24hrs prior and you will be able to alter your session or cancel up to 18hrs prior. Changes to your appointment with less than 18 hours notice may result in a $20 service fee that must be provided prior to further service. This policy is exempted during official snow emergencies.
No Call/No Show. No Call/No Show situations will be charged the full amount of the missed session prior to any future services being rendered. If there is a second occurrence, clients will be asked to place a valid credit card on file and be charged in advance of their appointment or at the time of scheduling. A first time client with no prior history of scheduling who No Call/No Shows their first appointment will be denied future service with no exceptions; please make every effort to contact if there is a need to miss your first appointment.
Inclement Weather. Any official road closings, government-issued travel restrictions, or official travel emergencies in the local service area will be respected and will absolve clients from any penalties due to missed appointments. Oswego Massage Therapy will make every effort to contact clients with pending appointments and alert them as early as possible to any office closings that are the result of inclement weather or other emergency.
Gift Certificates are non refundable, and cannot be exchanged or combined for or with other services. Gift Certificates for a specific service or session length cannot be broken down into different session lengths or used as partial payment for an alternative session length.
Specials. Unless otherwise specified, sales prices or special offers cannot be combined.
Returned Checks. There will be a $25 charge for all returned checks.
Health Intake forms must be completed prior to any service for first-time clients.
Minors between the ages of 13 and 18 years old may receive treatment with the written consent of a parent or legal guardian in addition to written approval from their physician. Oswego Massage Therapy does not provide treatment services to individuals under the age of 13. Ages 13-16 require the presence of the parent or legal guardian in the room at all times during treatment.
All client information is safeguarded and will not be shared with any third party at any time unless disclosure is required by law, required by court order or client has released written consent for insurance purposes. This includes both written and verbal communication, client records, and client schedules. Clients may receive copies of their records following written consent. Clients have the right to refuse, modify, or terminate treatment at any time regardless of prior consent. Oswego Massage Therapy does not discriminate or withhold services based on culture, ethnicity, age*, gender, belief, or sexual orientation.
*With the exception of uniformly restricting services to ages above 13 years.
Oswego Massage Therapy reserves the right to refuse, modify, or terminate treatment to any individual at any time. Clients will be refused service if there is suspicion that they are under the influence of drugs or alcohol.
